Care Coordinator Job Description (Non-Exempt)
Care coordinator is a well-rounded social worker who will be working with formerly unhoused seniors at a single site Housing First community. The care coordinator at Towne Twin Village will be a member of a multidisciplinary team that works together to aid residents in achieving their maximum potential. The role of the care coordinator is to work with residents in varying capacities to help them meet goals set by themselves, and to be part of a larger team that uses a Housing First model. Care coordinators must adhere to the Housing First philosophy including but not limited to; harm reduction practices, client led case management, client set goals, motivational interviewing, trauma informed care, and life skills practice.
Care coordinators will closely work with property managers, healthcare staff, administrators, and sometimes outside agencies to create the best experience for residents of Town Twin Village. The main role of the care coordinator is to facilitate resident success based on the goals that they set for themselves. A successful social worker in this role understands that this position reaches outside of the bounds of typical case management and embraces that there is no one size fits all approach that works. Tasks can range from standard charting and SOAP notes, working on cooking skills, or gardening together, so flexibility and an open mind are essential to this role.
Essential Job Functions
❖ Exhibit and model Housing First principles and embrace Towne Twin Village’s unique case management style.
❖ Build rapport and long-lasting relationships with residents both on and off of caseload.
❖ Perform assessments upon intake as well as periodic evaluations throughout the year and chart/ document findings appropriately and timely in HMIS software.
❖ Work with the resident to complete an ISP that is tailored to their goals and interests in a dignified manner.
This includes actively listening to the resident and letting them take the lead in determining what their care coordination needs are.
❖ Maintain a caseload of 10-15 residents depending on their level of need and meet with each on a regular basis that will vary with every resident.
❖ Use evidence-based practices such as Motivational Interviewing, Trauma Informed Care, and Harm
Reduction.
❖ Walking residents through the leasing process and facilitating their orientation/ move in process.
❖ Take an active role in community building in many aspects including resident programming.

Job Requirements
● Good Interpersonal and written communication skills.
● Knowledge of social work theories and Code of Ethics.
● Open attitude to adopting a new case management approach.
Knowledge, Skills, and Abilities
● LMSW from an accredited university.
● Bilingual (Spanish-speaking) preferred
● Experience with housing and or homelessness.
● Knowledge of principles and practices of organization and management, particularly of nonprofit organizations.
● Skill in using a personal computer and associated software programs.
● Ability to accurately collect and enter data.
● Ability to communicate clearly and effectively.
● Ability to perform responsible programs and office administration duties.
● Ability to conduct assessments, intake, evaluation, implementation of care plan, and service planning.
● Ability to understand the organization and operations of the Towne Twin Village and of partner agencies as necessary to assume assigned responsibilities.
● Ability to work independently in the absence of supervision.
Compensation
● $ 45,281, – 67,923 annual salary, with additional competitive benefits package. Pay range may vary depending upon skills and experience.
